Notice for brochure of the White Coat Ceremony
1. 如同上次班會報告的，所有人「必填」此連結的表單。
As the last meeting report, dear all MUST fill up the web form by the link.

2. 此表單會用在白袍手冊裡面，如果你「不參加」白袍典禮，則不用填此表。並請事先告知我。
The form you have filled up will be used for the ceremony brochure. If you won’t attend the ceremony, you don't have to fill the form. But please tell me in advance!!!
3. 每個人在手冊中，都有自己的頁面。如果你沒填表單，就只有你那頁空白，很怪吧！
Everyone has a page in the brochure. If you didn’t fill up the form, your blank page would ruin the brochure.
4. 除了你對自己的留言，對其他人的留言，可填可不填
Leaving messages to the others is optional, but writing a message to yourself is a must.
5. 對其他人的留言，可選擇是否匿名
Leaving messages for the others by anonymous is optional.
6. 若針對某些人想匿名，某些人不想匿名，請填兩次表單
If you want to leave a message for someone by anonymous and leave a message for the other by name, please fill up the form twice.
